Indian Chiefs are still permitted to opt out of Medicare if they judge wampum (or the regulatory burden) to be worse than sweat lodge ceremony. Up to now, this has chiefly been an issue with the more warlike tribes like the Apache (sturgeons). 7th Cavalry takes a very dim view of anyone leaving the reservation and won't let them back on for a whole year (last time I heard).
Medicaid is different as the states have to chip and the wampum is generally worse than 4 day old buffalo liver in August.
Chiefs who leave reservation have to provide adequate time and support for squaws and braves to find new Chief or risk an abandonment claim.
Your Chief may be opting for "Boutique practice" where he takes many fewer patients and patients sign a contract where they pay a goodly retainer up front and then came come in anytime for quick rectal exam or other fun things.
As for 7th Cavalry. It is now quite obvious they can do whatever they want

Not being of Londinium stock or heritage, I must confess ignorance in comparative cultural norms. What is the difference between an English "Open All Year" Peace Camp and illegal squatting? We would have classified that activity as the latter here in the colonies.

A. It's in Scotland and not England and
B. The "Peace Camp" was sponsored by the local council. Strange but true.

If this is the place I think it is, in NW England just across the border from Scotland, it was a place where Scottish couples eloped to for marriage because Scottish customs were too strict. The demonstrators in the pic may be seeking legalization of gay marriage, if that is not the case yet in Britain. Of course legalization of marijuana and protests against people defending their freedom to protest are also possible expalanations. Either way, if you have a Y chromosome, don't bring a case of beer and ask if they want to party ...[:-]
Edit: Correction - the place I was thinking of was Gretna Green, not Greenham Common.

Only Admiral Lord Sprior and I can post extraneous filler crap that has nothing to do with WWII(c)!
So, a year ago, after a couple of drinks, I bet this trauma doc $100 that his guy wouldn't win the Presidency. I lost the bet. He makes a TON of money and his wife is an internist...so they both have big targets on their foreheads. I think they both thought the other guy (who actually had not even been chosen yet when I made the bet) was going to ban the teaching of evolution in schools and make everyone wear "magic underwear".
His wife came in compalining a while back that her taxes were too high. I had to laugh out loud.
Anyway, in a generous gesture, he suggested I pay off the bet with a donation to the Wounded Warrior Project. That was classy. Just as well, because I was goign to send in a 1099 to the Feds with his gambling winnings.
No...turn...yet. <odd facial twitch starts to kick up>
