Grayed-out Ships on Screens

I know that on the screen where you transfer ships, a grayed-out ship can't be received (usually see this when a docked TF tries to add a ship that would be too much for the base's docking limit).
I had a Jap sub in need of major flotation repair dock at Truk, where an AR was in-harbor. When I specified repair type, readiness, pierside, and repairship were all "X". But now, when I try to put the sub into any sub TF, or even (desparation!) an escort TF, the sub is listed on the available ships screen (if repair is at readiness) but the sub is grayed out and clicking on it won't put it into any TFs. I can't fix it at Truk but I also can't float it home (it currently has 28 major flotation damage points).
What's happening here? I didn't find anything in the manual.
On a related theme, I had a sub low on fuel. WitP let us refuel subs at sea from other subs at sea; AE doesn't. Okay. So I pulled into a nearby port and was unable to refuel it there!! Size 2 port with lots of fuel. What's the deal here? I thought a port with fuel could execute a refuel? What did I miss?
