As I sail up and down the Baltic coastline, I see the wonderfully colourful Danish and Swedish counters (and the Spanish when sailing into / out of the Med). How, if at all, do these countries enter the game - if not directly attacked?

They don't. Maybe if you declared war on enough neutral nations others would join your enemies by choice, but it's not likely to happen.
