There is a patch that is dated 12/22/2014 that says it is a 1.08 patch and when applied to the game shows it as version 1.08

There is a beta patch 1.08.01 which is dated 12/13/2014.

So does the 12/22 beta include the .01 and the start screen just does not show it? Or does the .01 patch need to applied to the 12/22 patch? Or something else?

You have to apply the .01 patch.
I guess you have misread the date of the 1.08 patch. Should be 11/22 or 10/22.

All patches are cumulative so they have all updates from previous patches.
