If not in Blitzkrieg posture, how do units accumulate fatigue?

Hi,

They don't.

Only happens when in Blitzkrieg Posture. It's assumed that they aren't pushing flat out (and gaining the big bonuses) in other postures.

There are still levels of fatigue but this is built into the standard readiness mechanics.

Blitzkrieg fatigue is considered to be above and beyond this.

Cameron

Then, when I find any unit not in Blitzkrieg that is marked with Fatigue, what could be the cause?

It was in Blitzkreig for a while where it got the fatigue. You have to rest the Division to remove it.
Changing Posture only prevents it getting more fatigue, it does not remove any that it already had.

Hi,

Yep, Falke is correct.

The tempo of combat on the Eastern front was such that opportunities for rest (enough to remove fatigue) in the normal course of operations were rare.
