Spotting - Same Hex

I played this game a fair amount late in 2014 and then dropped it for others. About two weeks ago I decided to take it up again and started playing again.
Quick question re spotting. I was playing the Tutorial Scenario (which, by the way, could use some updating because a number of subsequent enhancements
greatly affect the play of the tutorial). At the end I found that a number of Soviet units were located in the same hex and 8in hexes adjacent to my fairly substantial units. I could not see them, except I would get a quick vision when I made air strikes after encountering fire from the adjacent hexes.
My questions are obvious. First, what leads to this situation - I presume the "accumulated visual hinderance" (Manual page 88).
Second - how do you tell when you are seeing not things that are in your hex or adjacent? (Fire indicators help some)
Third - what do you do about it? Does a unit need to assault into its own hex? How do you get to these units?
