Combat and Fleet Targeting question

I presume I should put a Combat Targeting system on every ship?
And wondering if I also need Fleet Targeting on each ship in a fleet?
Or is Fleet Targeting on one ship enough

Yes - you need to put "Combat Targeting" on every warship.
Any component of this nature that has "Fleet" in front of it just needs to be added to one ship in the fleet.
I like to create a special "Utility Ship" design that I use to load all these "Fleet" components onto. So long as I have one of this ship type in each fleet, I don't need to add those components to all the other ships in the fleet.

If you're going to make a specialized utility design for inclusion in your fleets, you might want to go ahead and just make it a dedicated command ship with heavy defenses and ensure that your fleet admirals end up on them. I'd suggest using the capital ship role for it; I have the impression that those are preferred for selection as a fleet's lead ship, and fleet admirals have a tendency to end up on the lead ship if you don't manually assign them to a specific ship.

I actually use the Escort design for my utility ship because I don't do escort missions, but Capital Ship works just as well. I manually assign all my personnel so it's never an issue.
I configure the ship as follows:
- all the "Fleet" components
- all the scanners (including long range)
- lots of shields and armour
- good engines and maneuvering thrusters
- one extremely long range weapon
- point defenses
- set posture to standoff from everything
