Proficiency check failed

Do I get it right that the Proficiency check is a 2 stage check, one based on the Force Proficiency and the other and how far the turn is advanced?
And there is no other way to fail the Proficiency check by failing both checks?
I just ask because I had a turn that ended after 40% with a proficiency of 90 and the next turn failed already after 20%. Seems like a lot bad luck.
