There are 2 images on the decision dialog box but I am not sure where to find them to modify them. Can someone point me in the right direction, please?

Within the game's main installation folder go to the Interface subfolder, and then it'll be in one of the Major subfolder, in this case it'll be like this:

Strategic Command - WWII War in Europe\Interface\Major_03

Hi. I am using WaW to mod but I have checked both it and WiE but there are no Major folders under Interface. I have Hidden Items ticked. What am I missing?

We've actually changed how these are implemented and the files you'll need to look at for those two images are the following:

Interface\game_screen_national_symbol_sprites.png
Interface\game_screen_flag_sprites.png

Essentially the #COUNTRY_ID owner of the decision event will have its corresponding national symbol and flag shown from these two files when the event displays in game.
