The Fate Stratagem Pack for getting to a Civ Level is awarded *each time* that civ level is achieved - which may happen multiple times. Should only happen when reaching that level for the first time.

In normal gameplay, this may happen when incorporating new cities (which may drop the level temporarily, then rise back up). For testing, it's easy to see this by just switching the regular/unincorporated setting on off.

I don't think Tech Level can go down, but this issue probably applies when reaching Admin Levels as well. Number of organization won't decrease, but amount of BP generated can fluctuate and cross some level threshold multiple times.

I think that this is also the case for conquering a city multiple times (if you lose it to a rebellion or enemy action).

That at least is not suppoed to be. The Handbook says:
"You will be awarded a free Fate Stratagem Pack (4 Stratagems) if the Zone conquered has 10K Populace or more. Note however that you’ll only receive these the first time you conquer a specific Zone."
