Can this be turned on during a game if omitted at the start ?

You and your opponent can both abstain from switching production factories into R&D ones, and it will effectively be a "Realistic R&D On" option. Opposite switch is also about player decisions.

However, if you started with realistic R&D Off, its largest effect happens in the first months of the game when Japan player is able to "repair" future R&D factories quickly by having them as production ones for the earlier models, and them switch them to R&D along the chain. So going to RR&D On is essentially no use, the job is done
