Vichy units supposedly have a chance to surrender when attacked by US or Free French... has anyone actually seen this occur in MWIF? And if so, what does it look like on screen. Frankly, I have either been very unlucky, misunderstood the rule, missed any relevant pop-ups, or it is not occurring at all, as I have never seen it happen. Does it happen for everyone else and if so, could you tell how it shows up on screen?

Well I didn't try in MWIF but for that to happen, Vichy must be at war, yes, but not hostile. It seems to be a friendly war, LOL

Since Vichy becomes hostile, for example, if you attack it in the Mainland, this rule would only work for attacks to Vichy minors and Territories. This way Vichy wouldnt be hostile even at war (unless hostile for other reasons which are listed).

There was some debate in my group on whether the first time you attack Vichy Mainland it had become hostile earlier (and so units fight) or it hadn't become hostile yet (only later) so, units could surrender.

But as I understand it, the rule is mainly for minors and territories.

Yes, it works as it supposed. Even if attacked by CW forces it can happen, but the odds are lower.

This was a bug in the earlier versions of MWIF, but it has been fixed since.

Which version are you using?

The 2nd latest hot patch, but it has been the .3s since circumstances have been such that the rule might be invoked.

Here is a game at the point of an attack from Free France against a Vichy unit. The game is on the 4.2.3.2 version of MWIF.
