In my current match (355405), my opponent never got the event to occupy Persia and it's summer 42 now. We're both kind of stumped, did we miss anything?

The Decision Event for Operation Countenance should fire in August 1941 (or at a later date when its conditions are met) providing:

Kuwait and Baku are in Allied hands, Persia is pro-Axis but neutral, and the UK and USSR are fully mobilized.

Could it be that one of these conditions aren't being met at the moment?

the conditions are met, that's why we're confused.
My Italians evacuated all of Africa early, there has been zero action in the Near East at all. Not even the Iraq uprising happened because I wasn't aware I had to trigger it
No diplomacy has happened in the Near East either.
In Russia I went for Leningrad and Moscow, none of my units is even remotely near Baku.

I've taken a look at your game, and DE 120 is no longer in the list of Decision Events yet to be presented.

You can see this yourself if you go to Options -> Advanced -> Scripts -> Decision

As the game progresses, Decisions that have been presented and are not repeatable are removed from this list, which suggests that it was presented and that the Allied player didn't notice and accidentally clicked no on it.

You can see this in action if you start the 1939 campaign in Hotseat mode, as you'll see that far more Decision events are shown there than in your game, because so many have already been presented.

In view of this, if it were my game I would recommend that out of fairness the Axis player declares war on Persia, so that the USSR can gain from the MPPs that the USA can deliver via Persia. But of course, I leave that decision up to you.
