As you can see - the colonies list shows all resources - even "undiscovered" ones:

I think it's enough to 'catch a glimpse' on debris ships/bases and add them to the list. This is one are where we do allow the player some hidden knowledge. You know when a location is completely explored and if it doesn't have much to find there, that could be early on at low exploration levels. That's not a bug.- Explorers can mark a system completely explored at only 10 Survey.
I don't see the issue here. You are assume to have a very low level of exploration of a system once you are there as with your home system. That doesn't mean those ruins are explored, just that you know they exist.- At the start of a game, click Explore Tab, Ruins, and see the ruins in your home system before scanning.
Not really an issue. You obviously know the base exists if the advisor is saying to send a constructor. You must have spotted it with another ship or through territory maps, diplomacy, etc. Knowing it exists then allows a mission to it to be queued. The discovery message is intended to only trigger when you first get within a certain distance of that location, not just because you know it exists.- Even worse than the example above. The resources show the presence even before the '?' appears.
That's a bug and was reported in the first post here in this thread as well.

If they do explore multiple Asteroids, the scanned and empty rocks should show a survey level, not a Zero. As well I have seen explorers bypass multiple asteroids.
