First of all, I am running version 1.04.00 of Decisive Campaigns Ardennes Offensive

have two supply related questions.

1. Under 4.4.2 page 75 of the manual - it says " A supply base can be set to five different modes." How do I change the mode? - where do I go to do that?

2. Under 4.4.1 on page 74 of the manual, it says that "the supply request % can be set for each HQ". Where is the % number shown that the HQ is requesting and where do I go to change it?

For the first one, click the hex where the supply base is. Check the panel on the right side of your screen. There you will find the hex information. There is a section regarding the supply base. You can hover your mouse over different options for the supply base status and just click the one you want to activate.

The supply request percentage can be determined by clicking the individual unit or HQ and again looking at the panel on the right (focus on the unit sub-tab - not the hex). There you will find the supply and reinforcement request options.
