I think it's still ion storm related.

I've ordered 2 fleets (6th and 8th) from Kappa Octanis to PP84 -- this is the blue arrow. Most of the ships go the right direction but 5-6 from 6th fleet go on a merry pursuit across the galaxy (shown in the yellow line: I have selected one of them (Evasive Nemesis)).

I think the reason could be a fleet with ships with different warp drives.

Yes, that was part of the problem. You found a nice edge-case there

Anyway, fixed in 1.1.6.5
