How to avoid "incident caused by reckless behavior"

Couldn't find any info from the manual. It looks like using cabinet retreat can reduce it, but even a non-partisan governor can get involved. Is it caused by the faction's profile, or is it just their bad relationships with me/others?

In case if zone events it depends on low zone statistics like described in the manual here:
5.3.16. ZONE EVENTS
Having certain statistics at low (or high) levels can have consequences in
terms of certain Events being triggered.

These fall in the main categories of Population Unrest Events, Worker
Unrest Events, Danger Events and Rebel Events.
High Health, Culture and Security scores can help protect you against some
of these Events.
In case if leader events I could only find some info that it could depend on low loyalty and relationship
Check manual section:
5.8.1.2. OTHER KEY STATS:
Low Loyalty (<50) combined with low Relation
has a big impact on the chance if any unsatisfied Leader events will occur
