Ancient Capital (Human) bug

Randomly the creation of the Sol System with Terra gets overwritten by placing an independent colony with a different name at the Continental Planet that is supposed to be Terra.
All items are still there > Ancient Terra Spaceport > Constitution Crystal, but it seems counterintuitive to have random independents placed at story event planets.
Seen this quite a few times now while testing some modification and restarting game often.
Happen this to the Ancient Human Capital event/creation I suspect all other races with same events (Ancient Capital creation) are affected by this too.
