I can see in some AAR that the republic can win here, but the Nationalist have some 70 AV compared to about 27 AV. So do the republic get reinforcements here the first turn or how is it possible?

The Republic has supplies and forts.

The Republic can win because there is no supply source for the Nationalists. Not on Day 1, but eventually. Republic gets more troops around there, but the big reason is that there is no supply source for the Nationalist units
The Nationalists are not strong enough to take the hex; their only choice is to hit the road to get to a supply source (or stay and starve I guess)

Also, the Republic can use naval bombardment to speed things up.

In the long run, it is extremely difficult for the Republic to defend against a human opponent for any length of time. The troops you get there in July/August are untrained militia. Meanwhile, the Army of Africa will be coming up the coast. It's also easy to outflank because the Nationalists should/will hold Grenada.
If the Nationalists abandon the hex July 17, it's only temporary
