On every atmospheric mission where I get destroyed the game will crash when it tries to load the mission selection menu.

Windows XP Pro, P4 3.06, 1 Gig RAM, ATI 9700 Pro, DX9.0b

This is on 4.0.2

The same happens to me, and it happens when a wingman is destroyed as well. I'm also running XP Pro SP1, with a 9700 non-pro, DX9.
