By adjusting difficulty, what changes? Does the AI get combat modifiers, diplomacy modifiers, better generals, etc. or does it actually make them more agressive on the strategic map?

IIRC at the highest levels I think it is mostly economic/production changes you will see.

No, the AI also gets combat modifiers at the higher levels, but the exact effects have always been a mystery.
