edit: It was American default rules, quik play CPU vs CPU, down and distance were obviously 1 and 10 since it happened on the opening kickoff, field position was around the 20 yard line of the receiving team, and I was using 2 default US teams (Tampa and Los Angeles)

I think you might need to give a little more information then just this. Can you be more specific on things like Down and Distance, Feild Position, Quarter Time, Teams played, Specific Rules used, any rules not used, any additional option rules used, Quick Play game versus League game?

In league play it happened to me with the 1.01001010110 patch. I had to waste a time out with 1:44 left in the game.
