Two minor errors?

1. In the ninth inning of a game ('66 Orioles vs. '66 tigers) - with the game tied, Jim Palmer loaded the bases. Eddie Watt then relieved Palmer and gave up a grand slam so Tigers won. Watt was credited with the loss although the winning run was Palmer's responsibility. I beleive Palmer should have been awarded the loss.
2. In the lineup screen, I changed pitchers (switched McNally for Hall) then changed my mind, without leaving the lineup screen, and replaced Hall by Miller. When I returned to the game, the play by play had listed both of these changes. I assumed that the any changes do not take effect until I leave the lineup screen, yet Hall was subsequently unavailable. If the changes take place as soon I make the change in the lineup screen, then I should not have been allowed to replace Hall as he had not pitched to any batters.
