When on a river hex are you considered to be across the river or in front of the river? Is it better to be defending in a river hex or in a hex behind a river hex?

Defend behind the river. Units that attack from a river hex suffer a penalty.

The rule book says "attacks from river hexes" suffer _insert penalty_. Thus, what I inferred is, to defend hexes that are "behind" the river hex, i.e., non-river hexes that are next to a river hex from whence your enemies must proceed.

Yes. The presence of a bridge only aids movement.
