I can't seem to pin this down as to what is causing it, but I have had this issue with several maps, with several differing amounts of models (from few to many)- the cursor will suddenly be unable to be placed directly on a given model, but still moves it. When "g" is hit to place a model, it disappears.

It seems that the "Y" axis is somehow reset. I just checked the file I am working on, and it seems that the model was around -10 below the ground level.
