Getting constant news spam about enemy leaders being killed.

Must be something in your games makeup or setting or something? Probably played 50+ hours in this last month trying alternate strategies, and don't recall even seeing those at all.

Tanaka - are You by any chances invading enemys homeworld, or bombarding it?

As hinted at above, whether normal or a bug depends on what is going on. If someone is bombarding a capital, leaders die a lot. If "spamming a lot" is 50 times a second and continuously, it is a bug :)

No bombarding or invading. Early in an age of shadows game so no one else can really do that either. 50 times a second no. Once every minute or so yes. If you look at the messages every time it says Galactic News that is a leader killed announcement!

Your speed setting is on 4x, that will add to constant bombing of messages, is this a custom theme by chance? If not then the AI has fluked a ninja as ASHBERY76 has pointed out.

I am only using the DW Extended Mod. Yeah sped things up because of research problems but even at slower speeds it still seems like excessive leader butchering. I am assuming it is a pirate race doing this? My leader seems to be the only one immune to this...

I see now that one pirate faction has 50 successful agent actions and I suspect they have the unstoppable assassin. Is there not any way for me to assassinate the assassin? All I see is assassinate leader?

You could try to find and kill the whole faction if you have enough ship power. If they are not close, you probably have to use agents to steal their territory map to find their base(s) and construction ship(s). I'd bet you would be quite popular with the other factions after killing off that assassin (maybe not in game terms though).

Regarding weird incidents, I had a sand slug constantly respawn every now and then in my latest game. It always popped up close to one of my planets and got killed by defense.

That wasn't by chance a super rare spice desert world. If so it is intentional and a Dune reference (which is why I renamed the resource Melange). The worms are the spice; The spice is the worms.
