I haven't had anything that bad but I have had ships hit reefs.

The real Kaga hit a rock in the DEI in 1942 and had to go for repairs.

I have had ships going up the inside passage and along the outside of the Great Barrier Reef (Australia) "hit rocks" or be "grounded" for several turns.
Amphib landings frequently have transports hit rocks.
But PTs are supposed to be able to pass over reefs (they can do that over the reef that connects Ceylon to India). Rocks are not reefs so I guess the issue is that the PTs are in designated shallow water where there is always a chance of hitting rocks or grounding.

Better start assigning some of these captains to land duty. With their penchant for grounding, it would seem that's where they want to be.

Can't say I have. I'd be curious to know if anyone had ever had a "Mutsu" incident where a major warship blows up by domestic mishap and enemy action is not involved.

IIRC, someone had a BB or other major warship lost to an "Accident" while in drydock. No fire mentioned so I suppose this is the ship slipping off blocks or losing its beams that hold it upright along each side of the ship.

I think AI had some incident on drydock in one of my games.

So yes, it can happen.

No Mutsu incident yet but I did lose 3 tankers to careless smoking in my last game, 2 near Los Angeles and one near Colombo
Plenty of groundings and losses but only during amphibious assaults

Collisions in big task forces are quite common.
