They are either core possessions or the 'Jewel in the Crown' that is heavily defended.

Note, however, they may be taken as "loot" if you win a war and have sufficient points to do so.

As for me, I wouldn't. If you take those ten points as reparations, you get a whopping 1000 points added directly to your economic base. Sicily will add about 300, and Italy will resent it, going forward. (The Railroads/highways event options add 200.)
