Adding new commanders ?

What files do you need to edit to add new commanders from scratch, I have tried and my all new commanders do not appear in game, Example if I add Ludendorff for Germany, Instead off placing at the bottom of the list number 50, Does he need to be inserted beside all the other German commanders, with everyone else below shifting down a position?
