Does Italy pretty much always surrender after the loss of Sicily ?

not necessarily, it depends on how many CV you have in mainland Italy and the date. never surrenders before august.

In my last 'test' game, I invaded Sicily first turn then transferred everything except a couple divs of Yanks to England for a winter invasion of France. Much to my surprise, in October Italy surrendered.
