When both sides have subs in a sea area (e.g., China Sea) the commit sub form doesn't show for the second side. By accident, or out of desperation, I found a work around is to save the game at the point when the second form should have appeared and then reopen it, at which time the form appears. Note, this is a case of the form NOT being there versus just hidden behind another form.
