Does the Physical Edition come with a Disc that has the game or is the game just digital only?

The Physical Edition is an add-on to the Download version. You will get both if you order it. You get the download right away, then when available the Hardbound book will ship out to you and it does include a personalized DVD in a sleeve attached to the inside cover with your name and serial number as well.

Does DVD include install of the game to keep as backup?

Yes, it has the full installer along with your serial number printed on it so that no matter what, you'll have a copy to install from (as long as you don't lose the DVD).
