Port defense?

What determines whether a port gets the 3x defense bonus?
For example, Port Moresby, Manila, and Suva (Fiji) all get it, but only Manila is labeled an urban port. Singapore does not get this bonus.

Ports size 5+ 100% protect a fleet from surface attack.

Port size 1-4 get some port defense to help against surface attacks.

You pretty much only want to use ports size 5+ for fleets.

Ground defense, not naval. There is the -66% attacker/+2.97x defender bonus on only certain port hexes and does not coincide with the urban status of the port.
