Well I have had (already!) a 90+ Japanese carrier ace being lost in a landing accident on the Soryu.  Also did it 9 system damage to boot as well!
Haven't yet had the case of torpedoing one of my own ships, but I guess in theory that can happen too...

Yeah, those noncombat casualties are so unfair.  I mean, who'd expect to see something like "KGV collides with destroyer" or "Illustrious runs aground" or "Mogami collides with Mikuma" or something like that?  Those are just so rare as to be ridiculous!
