You have to research the Find Wormholes techs (Large -> Medium -> All). Then, to be able to use them, you have to research the Use Wormholes techs.

I did but how I do I know where the wormholes are? Do I have to re-explore every planet? Try to travel through one?

They will show up on the map once you have the tech to see them. You then move on to them to move to the other side. Actually, I think you might have to move to it, then move to it again to use it, I'm not sure.

They also need to be in scanner range, so either a ship passing close by or by one of your scanner arrays on a planet

Yeah, it seems it was the scanner thing I didn't get. I rarely build scanners and just use Hypersense. Looks like I'll need some survey ships at least [:-]

^ I usually build planetary scanners -- partially because, as just mentioned, they can see wormholes (once you have the requisite technology), and partially because you can build them right away (whereas Hypersense takes a bit of research to get).
