ACH-47A Chinook, four built, one surviving "Guns-A-Go-Go" which is the likely source of the picture
XM159B or XM159C 19-tube 2.75" rocket launcher and a M24A1 20mm cannon mounted on XM34
I am not sure about the door mounted gun, perhaps a M24 mounted M60 or like

The "Give this one A GO" was a hint. The window mounted gun is a .50 cal.
