Alright, does this actually do anything? I pay the money as France......nothing special happens.

it does do something, it improves how the other nations look at you

if you don't pay it, it makes you look that much more in the hole

now, is it worth it ?

depends, but for the most part, you going to have relations in the -1000/4000 range pretty quick (if France) not sure how much a 125 buck payment is going to help, or hurt

if I got the money, I give it, if I don't, oh well, nobody likes me anyway

Does that event fire even if you've annexed the Papal States? That would be a bit odd. In reality, the French were extracting payment and paintings from Rome....
