I believe the division leaders mod from CrashDavis' website lets you do this.  However, I may be mistaken.

Yes, career ending injuries do still happen.  I've had a couple of them in my league (thankfully none to any of my players)

It does indeed make since. All a pitcher needs to do to pick up a win is be the pitcher at the time your team takes the final lead.
