Some suggestions to fix them:
1. Reduce building limits of rifle brigades.
2. Enforce building limits after they get reduced as the time progresses.
3. Limit the amount brigades/support units in general that can be directly assigned to Rifle Corps to 1.

You don't pay a premium for building them yourself. Look it up, it's in the manual. IIRC AI costs 10 AP and will build 10 depots. You can build them wherever you need them without disbanding the ones back in the line. AI management should never be used.

The event for the 2nd Polish Army cant be triggered before January 1st 1943, both in the early end as well as the no early end campaign.

I'm pretty sure I did not displace 50 HQs, maybe 20 at max

15% of 20 means you'd assume to lose/kill 3. The list shows 4. So if your guessed 20 displacements are correct, the 15% the Devs chose as an chance I could imagine to see in this example.

I'm not sure how the game calculates the odds for tripple stacked HQs, I often felt like I would kill all three leaders there.

As it was a server game, I cannot access the older turns and my memory isn't the best. There certainly were some issues with HQ placements, but most german leaders died ...

I took a deep dive into the manual to understand and summarize which parts of the game are influenced by leaders stats. In brackets you can find the corresponding section of the manual to easily find the sources of information.
